Who you are:

The Director of Care Transformation is a razor-sharp, healthcare nerd for justice who is excited to help client organizations build Community Health Worker programs. You can synthesize new information quickly and ideate on your feet to solve problems. You can scale-shift between high-level vision and operational details. You are an excellent listener, but also have enough gravitas to command a room of senior executives. Your approach to program building is data-driven and patient. Your clients love you because you meet them where they are and always do what you say you are going to do.

What you’ll do:

You will use IMPaCT’s evidence-based infrastructure to help your client organizations plan, build and support Community Health Worker programs at enterprise scale. You will be the account owner for every stage of the client journey:

  • First, you’ll lead a series of strategic planning conversations. You will help clients and stakeholders articulate their vision for a Community Health Worker program. You will then work backwards from that vision to an operational plan. You will do a gap analysis of any existing Community Health Worker programs, define key metrics, build financial models, identify priority populations, and define program roles and workflows, including the flow of information between the electronic health record and IMPaCT application.
  • Once you have aligned on a plan, you will help your client execute it; you will use IMPaCT’s evidence-based infrastructure to hire and train empathic, trustworthy Community Health Workers who come from within the communities they serve.
  • You will partner with our technology team to help organizations optimize their use of IMPaCT’s cloud-based application, including supporting electronic health record integration.
  • You will stay closely engaged as your client builds their Community Health Worker program, coaching them on topics ranging from clinical integration to career ladders to stakeholder engagement.
  • You will use IMPaCT’s technology and analytics to run continuous improvement cycles; you will ensure that your client’s Community Health Worker program is delivering magic with consistency at scale.



What you bring:

  • Graduate Degree in Health Policy, Healthcare Management, Population Health or Healthcare Administration
  • 5 years of healthcare experience including healthcare consulting, health policy or health system leadership.
  • In-depth understanding of value-based care, health information exchange and Medicare and Medicaid financing
  • Exceptional presentation, interpersonal, and writing skills
